In 1945, Truman of America decided that he should not drop the bomb...yet. In August 9, 1945, the Soviets overran Manchuria and invaded Hokkaido the next month. The US have already invaded invaded Kyushu when news that the US will drop the atom bomb in Japanese cities caused a stir in Tokyo. So in October 8, 1945, the Emperor surrendered. Japan was divided into American and Russian zones. Manchukuo mutated into a Soviet Socialist Republic of Russia, so most China Proper save Taiwan became communist. Korea was divided similarly to Japan. The Soviet Zone became the People's Republic of Japan. Japan, China, and Korea, and eventually also Vietnam have two governments, one commie and one democratic. But basically in all four communist iteration of the states, even in North Korea, limited capitalism is the rule. All of them are still police states, of course.
The Emperor remains the head of state of South Japan.
North Korea mellowed down with the death of Kim Il-Sung in 1984, with blackmail from North Japan. Kim Jong-il is still the President, but a complete figurehead. North Korea then retooled Juche to mean improvement of living standards with whatever support it could get. North Korean pop culture is slightly more conservative version of its southern counterpart, with more limited president-worship than in OTL.
But in each divided country except in China, the economies of the communist governments are much smaller.
North Japan is most likely resemble an Asian Yugoslavia under Tito. Korean and Ainu are secondary national langauges as well. As in the South, tons of anime are produced. Fewer porn unlike in the south, though.

In the alternate timeline, where General Koopa was inspired by the characters of the socialist version of Mario Bros. (Squirrel and Hedgehog style) made in North Japan. General Koopa: He is the general of the Koopa Armed Forces, they are capitalists. He is the main villain of video games. with his military suit of the combination between Mexican military and ancient Aztec warrior. He is one of the few characters who knows that the communist Mario is a spy for the Red Mushroom Palace.
